Essential Duties & Responsibilities include (but not limited to):
- Evaluates the need for medical social casework services to patients in their place of residence.
- Communicates patient needs to the physician and home health care team.
- Plans quality medical social work services.
- Implements social work services to patient in their place of residence.
- Evaluates medical social work services plan on an on-going basis.
- Maintains assessment and progress documentation accurately.
- Participates in the quality improvement program.
- Master Degree in Social Work from a school accredited by the Council on Social Work Education.
- Minimum one year social work experience in a health care setting.
- Previous experience in a licensed home health agency preferred.
- Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
$37.00 - $50.00 hourly
